Fitting and Evaluating a Hearing Aid for Recipients of a Unilateral Cochlear Implant: The NAL Approach

چکیده

32 THE HEARING REVIEW AUGUST 2004 reach the right ear a bit sooner than it will reach the left ear. It will also be slightly louder in the right than in the left ear. This head diffraction effect will enhance speech intelligibility in noise when speech and noise come from different sides of the head, because the brain can selectively attend to the ear with a better signal-to-noise ratio (SNR) for understanding speech. Even when the same signal and noise reach both ears, the brain can combine both inputs to produce more salient central representations of the speech signal (binaural redundancy) than if only input from one ear is available. The brain can also make use of the inter-aural time differences to partially reduce the deleterious effect of noise (binaural squelch). For unilateral cochlear implant users who have residual hearing in the nonimplanted ear, it is possible to provide binaural hearing by fitting a hearing aid to that ear. Increasing evidence points to the binaural benefits that are possible with the use of bimodal hearing devices, including improved speech intelligibility, better sound localization, and functional improvements in real-life environments. This article examines this research and discusses clinical issues related to bimodal device use.

Fitting and Evaluating a Hearing Aid for Recipients of a Unilateral Cochlear Implant: The NAL Approach Part 1. Hearing Aid Prescription, Adjustment, and Evaluation

Research on Amplification Requirements and Validation In conjunction with the Children’s Cochlear Implant Centre in Sydney, NAL investigated the amplification requirements of 16 children who received unilateral cochlear implants and continued to use a hearing aid in the non-implanted ear. This research determined the best frequency response for understanding speech by using a paired-comparisons...

Binaural-bimodal fitting or bilateral implantation for managing severe to profound deafness: a review.

There are now many recipients of unilateral cochlear implants who have usable residual hearing in the non-implanted ear. To avoid auditory deprivation and to provide binaural hearing, a hearing aid or a second cochlear implant can be fitted to that ear. This article addresses the question of whether better binaural hearing can be achieved with binaural/bimodal fitting (combining a cochlear impl...

Recognition and localization of speech by adult cochlear implant recipients wearing a digital hearing aid in the nonimplanted ear (bimodal hearing).

BACKGROUND The use of bilateral amplification is now common clinical practice for hearing aid users but not for cochlear implant recipients. In the past, most cochlear implant recipients were implanted in one ear and wore only a monaural cochlear implant processor. There has been recent interest in benefits arising from bilateral stimulation that may be present for cochlear implant recipients. ...

Exploring the factors influencing discontinued hearing aid use in patients with unilateral cochlear implants.

Studies have shown that unilateral cochlear implant users who have residual hearing in the contralateral ear can benefit from combining a hearing aid in the nonimplanted ear with their cochlear implant. The purpose of this study was to better understand the factors influencing decision making by adults. Adults who had discontinued hearing aid use shortly after cochlear implantation were selecte...

Hearing-aid fitting in profoundly hearing-impaired children. Comparison of prescription rules.

Generally, the performance of a hearing-impaired child with his or her hearing aids is the major criterion in selection programmes for cochlear implantation. Thereto, it has to be considered whether the hearing-aid fitting is optimal. For this purpose, methods which prescribe hearing-aid gain are valuable, especially in young preverbal children. Three of these methods were evaluated by comparin...
